As for a custom title, it is bestowed on you…

No, as your post count increases your "rank" increases. That is why you see people with different "ranks." Maximus for example is currently an "airman," others may be a "Sergeant" or a "Colonel."

The people with custom titles that are not ranks had those names given to them. Usually for something they have said or done.

Remember, though, the point is not to just post to increase your "rank."
