I am sorry but it looks like a basic error. I was always told that when formation flying, if you lose sight of the other aircraft always immediately take a safe action to leave the area, no exceptions.
 
Complicated formation flying being done by pilots that may only practice a few times a year is not a good recipe. Formation flying is not that hard to do...if you practice, know the rules, and fly within your limits. I don't care who trained you or what level of experience you have attained it still takes practice and currency to be safe. A static line is suggested because he hit his head on the tail. Had he not been wearing a helmet he could have been knocked unconscience and not able to pull the ripcord.
