With all these videos and news we learn how Ukraine's technological advances are killing more Russians. But we never see reports of Ukrainian ground offensives taking back any sizeable territory. If they're killing Russians at such an increasing rate, surely there's less of them on defence?

There's a lot more to successful warmaking than simple body-counts. Surely you're aware of the Vietnam War?
 
It's not so simple. A couple of points:
  1. The Russian's have a far greater number of people to draw on: Russia's population is approx 4.5 times larger than Ukraine's. The Russian's are also using conscript labour vs Ukraine's volunteer force so while the "meat grinder" is chewing up Russians, there are many more to feed into it.
  2. Ukraine is also operating on a "shoestring" when it comes to resources. Despite all they have managed to accomplish with drones and the like, and despite the support they have been given, they still lack heaps in terms of equipment to launch an offensive. Also often the support has been trickle fed to them - e.g. the tanks/fighters.
  3. The front lines are pretty stagnate - much like WW1 - so while a lot of killing is going on, very little ground is being taken, but either side.
  4. Both sides are heavily using drones and the Ukrainians are taking losses too - unfortunately, these hurt the Ukrainians more
  5. Even if Ukraine did launch an offensive, they need to make sure they have the support of other nations to sustain it and this isn't always forthcoming
  6. The Ukrainians have also been hurt by leaks of intel' (from certain Americans) and thus are probably doubly cautious about this happening again.
Overall, while there are a lot of individual Ukrainian actions that are shown, there is a difference between the individual/tactical level and the big picture/strategic level. Ukraine can still win dozens of engagements and still just be ground down by the larger numbers.

Well yes, and the Americans drove up massive body-counts in the Vietnam War, killing an estimated eight hundred thousand NVA/VC (about 3–4 times U.S. combat deaths), and yet they still lost. That's mainly my point. From 2022 to present, Ukraine has reportedly killed up to 362k Russian soldiers (much like the Americans in Vietnam, that's about 3-4 times Ukraine's combat deaths), but cannot seemingly take any ground, much like the Americans in Vietnam. Your analogy is a good one, in that a huge body count does not always equate victory, or even give the ability to advance.
