Assigned to 133rd Assault Support Helicopter Company.
Assigned to B Company, 228th Assault Support Helicopter Battalion "Longhorn".
Accident on the 29th January '66 5 miles north of Phu Cat, South Vietnam. The forward transmission froze in-flight while transporting a sling load of JP-4 fuel and the aircraft crashed inverted.
The entire crew of five perished in the crash.
