It might, depending on the fallout pattern. If radioactive material enters a NATO action's territory, then it could well trigger Article 5.
It rather depends on how strongly the NATO nations react. There is a scenario where Russia apologizes and pays reparations to the affected countries. I suspect that won't satisfy the hawks within NATO, particularly Poland, the Baltic nations, Finland, Sweden, the UK….but others may be more willing to fold (e.g. Turkey).
