If the blood supply is restored does the head of the femur regenerate? The first x rays I saw after being taken out of the cast looked worryingly out of roundness.
 
If there is no foveal artery as there is in the elderly then the head has no blood supply and will die and degenerate if the fracture in the neck of the femur.

If the fracture is intertrochanteric or subtrochanteric then the blood supply to the head is preserved so operations such as the dynamic hip screw, x-bolt or cannula red screws can be used to fix the fracture and preserve the femoral head.

All intracapsular fractures (in the surgical neck) will need hemiaethroplasty because of the loss of blood supply to the femoral head which will cause avascular necrosis if left. This causes pain and reduced movement which would need replacement either with a hemiarthroplasty or a total hip replacement.
