Narrative: Westland Lysander Mk.I L4813 (OO-R) 13 Sqaudron, RAF: Written off (destroyed) when lost (failed to return) from combat operations on 15 May 1940. According to the official Air Ministry file on the incident (File AIR 81/8367): "Leading Aircraftman C F Lucas and Pilot Officer A C Ollerenshaw, report of deaths; Lysander 51260 crashed at Louvain near Brussels, Belgium, on the 15 of May 1940.

Operation: Tactical Reconnaissance over the Dyle Line. Crashed at 10.00 hrs (Belgian Time, GMT +1hr) into the Meerdaalwoud, a few meters from the Limietendreef at Bierbeek, near Leuven (Louvain). Exact cause unknown, but possibly shot down by Oberlt Fronhöfer of 9./JG26. Both crew killed.

Written off (damaged beyond repair); crashed into hedge when undershot on landing at night at RAF Ansty, 5 miles East of Coventry, Warwickshire 08/03/40. Both crew - Pilot Officer F T Cragg (Instructor) and Leading Aircraftman N M Maynard (Pilot Under Training) - slightly injured.

Not repaired, deemed "damaged beyond econmic rerpair", and Struck off charge 07/06/40 as FACE (Flying Accident Cat.E).

De Havilland DH.82A Tiger Moth MSN 83540; Built by Morris Motors at Cowley, Oxford. Taken on charge as T7113 at 5 MU RAF Kemble, Gloucestershire 11/07/40. To 16 OTU RAF Upper Heyford, Oxfordshire 29/10/40. Returned to 5 MU RAF Kemble 13/02/41. To 24 EFTS RAF Luton, Bedfordshire 21/05/41.

Written off in a Mid-air collision with Miles Magister L5984 (also of 25 EFTS) and crashed near Luton, Bedfordshire 07/07/41; three fatalities involved in the collision - one in the Tiger Moth and two in the Miles Magister

Written off when aircraft spun into the ground 1 mile South West of Balzac, Alberta 01/10/41; Had 147:15 hours logged time on airframe when written off. Pilot - Leading Aircraftsman J R Whittaker - seriously injured.

To No. 10 Repair Depot for write off. Struck off charge 03/11/41 and broken up for spares.

Balzac is a hamlet in Rocky View County, which is in the Calgary Metropolitan Region of the Canadian province of Alberta. It is located immediately west of Queen Elizabeth II Highway, at the intersection with Highway 566, 24 km north of Calgary city centre and 12 km south of Airdrie.

Narrative: De Havilland DH.82A Tiger Moth MSN 3789 (Gipsy Major #81482) C of A 6470 issued 15/12/38 to The Air Survey & Transport Co Ltd. Registered ZK-AHA (C of R 154) 03/03/39 (or 24/02/39) to Otago Aero Club, Dunedin (based Taieri). Taken on charge by 1 EFTS Taieri 16/10/39 & formally impressed as NZ713 24/10/39 (at a cost of £1,250) Still at 1 EFTS (11/40). Undercarriage written off by LAC McNamara in heavy landing at Taieri 26/09/41; repaired. With 1 EFTS (10.42).

On 19/10/42, Tiger Moth NZ713 was being operated by No. 1 Elementary Flying Training School when a pupil pilot fell out of the aircraft during a slow roll. There is no fatality recorded in this accident report, so the trainee airman must have saved himself by the use of his parachute. As one would expect the flying instructor in the front seat landed the aircraft safely.

Sold (free issue) 4.9.46 to Marlborough Aero Club. Registered ZK-AKJ 31/07/46 (or 02/09/46) to Marlborough Aero Club, Blenheim. Spin in and crashed/destroyed by fire Omaka Aerodrome, Blenheim 01/12/46 during aerobatic display; pilot killed. (See separate entry). Registration ZK-AKJ cancelled 18/02/47.
