IMHO and AFAIK the above photo must be staged. The a/c from the SHINTEN SEKUTAI (Air Superiority Squadron=kamikaze unit) marked by that double-headed arrow, did not have a rear gunner and/or other armament. Unless this is the pilot going to his place and the rear hatch is just opened for unknown reason.
Here's a photo of another a/c from the same unit - check rear cockpit:

Translation (ChatGPT):
November 1944 (Shōwa 19). Photograph taken shortly after the formation of the 53rd Hiko Sentai's, 3rd Shinten Sekutai. before its personnel strength was increased to eight men, together with a Ki-45 special-configuration aircraft.
From the left: Sergeant Major Tanoue Hisago, Second Lieutenant Aoki Tetsurō, Sergeant Major Yamada Kenji, and Warrant Officer Iizuka Shigeo.
The aircraft in the background bears a red fuselage identification band with a white border crossing the unit's arrow emblem, and is believed to be the aircraft of unit commander 2nd Lt. Aoki Tetsurō.
