The company that built the TIROS weather satellites only had one of the carts they used to work on the spacecraft. They even had to ship the cart it to Vandenberg AFB in order to be able to prep the spacecraft before launch. They were also using the cart to work on another type of spacecraft during the 2nd shift and thus would remove the bolts holding the TIROS to the cart and set the spacecraft aside so the cart could be used for the other one. Then when finished they would have to move the TIROS back to the cart. And one night they forgot to put the bolts back in. The next morning the day shift came in and started to work on the TIROS. The $50M spacecraft was totaled.
