The company that built the TIROS weather satellites only had one of the carts they used to work on the spacecraft. They even had to ship the cart it to Vandenberg AFB in order to be able to prep the spacecraft before launch. They were also using the cart to work on another type of spacecraft during the 2nd shift and thus would remove the bolts holding the TIROS to the cart and set the spacecraft aside so the cart could be used for the other one. Then when finished they would have to move the TIROS back to the cart. And one night they forgot to put the bolts back in. The next morning the day shift came in and started to work on the TIROS. The $50M spacecraft was totaled.

A perfect example of False economy 101.

When I was at Tinker AFB I heard of a case that occurred up at Boeing Wichita.

One shift was working on a fuselage half and found they could not do their work because the wiring had already been installed. So they used their entire shift de-installing the wiring. The next shift came in and found their wiring had been removed and spent their whole shift putting it back in Guess what occurred when the next shift came in? Yep! Took it back out!

This went on for a whole week, the workers wondering if anyone in management knew about it. Finally they broke out of the endless Do-Loop and assumed that someone had finally woken up.
