Some by choice (we go to a doctor that charges a little over double the "schedule" fee because he is good and a 10:00 appointment means you are talking to him by 10:01, unless there is an emergency (once in five years). At bulk bill practices a 10:00 appointment can mean you see the doctor at midday or later.
